The office is open Monday through Friday from 8:30 a.m. until 5:00 p.m. by appointment. If you have an urgent condition, arrangements can generally be made to see the doctor promptly.
The doctors work on a schedule. It is important, therefore, that you arrive in ample time for your scheduled appointment. Should you be unable to meet your appointment, kindly call the office as far in advance as possible. We will help you with rescheduling.
Because we occasionally are called upon to provide emergency services, we sometimes may run behind or it may even become necessary to reschedule your appointment. We regret any inconvenience to you and will always try to honor the time reserved exclusively for you.back to top
Although the office operates on a scheduled basis, we are available 24 hours a day, seven days a week. Should you experience an emergency, we provide a round-the-clock answering service. The forms that are listed on this website are in one of two different formats, either written in Adobe Acrobat Format, pdf type form, or "On-Line" html web forms format. To see and read the Adobe pdf format forms, you need to have Adobe Acrobat Reader installed locally on your computer to read these forms. Some of the pdf Adobe forms can be filled out and then click the send button at the bottom of the form to send the form "On-Line". Utilizing a type of e-mail system, or you can print it out and fill all the information in and fax or bring it to the office. There are several "On-Line" type html web forms that you can fill out and then click the send button at the bottom of the form and it will be sent to the Office by a type of e-mail system. In many cases medical or dental insurance pays a portion of the investment in surgical care. Insurance is generally designed to assist you with the expense, not eliminate it. Our office will assist you in determining your specific benefits and, if you wish, will file your insurance claim for you. We expect the portion of your bill not covered by your insurance to be paid on the day of services.
If you have any uncertainty about the extent of your coverage, we recommend preauthorization as a method of determining benefits. This can be done at your consultation appointment.
It is important for you to know that you are covered as fully by your insurance if surgery is performed at our office as you would be if surgery were performed at a hospital.
Please bring all insurance information with you, including both your medical and dental cards.back to top
